Do I need a photo identification?

Yes, you will need to provide a valid photo identification when scheduling your psychiatric evaluation. Acceptable forms of identification include a driver's license, state license, or passport. 

If the evaluation is for a child, we will also need the parent/guardian's identification and the child/patient's identification, such as a birth certificate or passport. Additionally, if parents are separated, we will require shared or sole custody legal decision-making paperwork. 

Please note that we only accept a valid, unexpired photo ID for legal and identity verification purposes. It's worth noting that pharmacies typically require in-state photo identification to dispense ADHD medication.
